This policy explains how SEVA Healthcare (“we”) handles information collected on SEVA Healthcare.
What we collect
- Appointment requests: your name, phone number, email address, nearest clinic, reason for your visit, an optional preferred date, and the time you gave consent to be contacted.
- Attribution data: the page you booked from and, if you arrived from an ad, the campaign source (UTM tags, gclid, fbclid). These contain no health information.
- Security data: a one-way hash of your IP address, used only for rate limiting and spam prevention.
- Analytics (only with your consent): anonymous usage data through Google Analytics via Google Tag Manager. We never send your name, contact details or reason for visit to analytics or advertising tools.
How we use it
We use appointment requests only to contact you and schedule your visit. Requests are stored securely and sent to our patient-scheduling system. We do not sell your information.

- Our email provider
- Cloudflare Turnstile (spam protection), if enabled
- Google (Tag Manager / Analytics) and Meta, only if you accept non-essential cookies
How long we keep it
Appointment requests are deleted from this website’s database after 12 months. Records that become part of your medical chart are kept as required by law.
Your choices and rights
You can ask to access, correct or delete the information you submitted, and you can change your cookie choices at any time using “Cookie settings” in the footer. Email info@sevamedcare.com or call (918) 935-3240. We will verify your identity before acting on a request.
Health information
Once you become a patient, your health information is protected under HIPAA and our Notice of Privacy Practices, which the clinic provides at your first visit and on request.
